Patient Information
Appointment Policies & Office Hours
Our regular office hours are Monday through Friday from 9:00 a.m. to 4:00 p.m. The office is closed for lunch from 12 - 1:30 p.m. Dr. Page typically has 2 days of scheduled office time during the week as well as 2 days of scheduled surgery time. Due to the nature of the specialty, Dr. Page may be called to the hospital at anytime. If Dr. Page is on call the day of your appointment, the receptionist will inform you ahead of time. Should Dr. Page get called out, we will do our best to reschedule you in a timely manner.
Please arrive at your appointment at the scheduled appointment time. You do not need to arrive earlier than this time, as we have allowed time for any paperwork or billing that needs to be done when you arrive. If you are unable to keep your appointment, please call our office promptly so we may best accommodate you and our other patients. This serves as notification in advance that if you miss your appointment and do not call to cancel or reschedule, a fee of $25.00 will be charged and billed to you. If you are a MediCal patient and miss your appointment and do not call to cancel or reschedule, no future appointments will be made for you and your referring doctor will be notified.
Telephone Calls & Messages
When you call during regular office hours, a member of our staff can typically answer any questions you may have. Phone calls, unless urgent, will be directed to the appropriate individual’s voicemail. Should you need to speak to Dr. Page, the medical assistant will take down your information and relay the message to her. A return call will be made to you as soon as possible.
Financial Policies
If you have insurance, please present your card/cards to the receptionist at the time of service. Copays and deductibles are due at the time of service. We will bill any insurance, however, we are not participating with all of them.
If you do not have any insurance, please contact us immediately so that we can discuss payment arrangements prior to your appointment. We accept cash, checks and most credit cards. We are happy to discuss any questions you may have about your billings. If your balance exceeds 90 days after the date of service, an interest fee will be applied. If you miss a monthly payment, a late fee will be applied.
Patient Paperwork
Our office recently upgraded to electronic medical records and we therefore ask all new patients to please fill out their paperwork prior to coming in for their appointment. You can download the appropriate forms below, and we ask that you also bring a “current” list of any medications you are taking, with dosages. Please wait to fill out the paperwork until 1-2 days prior to your appointment so we have your most current and accurate information. We know many people’s medications, allergies and medical history can change daily so we want to be sure we have the most accurate information about you.
Please also be aware that, as part of your electronic chart, we will be taking a photo of you and copying a photo I.D. at your first visit. This is for your safety and privacy. Disability Form Assistance
We will be happy to complete the physician portion of any state disability form for a fee of $20 for the first page. Please note that the physician has to sign the form so there could be delay in mailing it, if the physician is unavailable right away. The typical turn around time is approximately 5-7 working days. We apologize if this causes any inconvenience.
Prescription Refills
When you need a prescription refilled, please call your pharmacy and they will fax or contact our office for approval. Please allow 2-4 business days for the physician to approve this, as they may not be available right away. Refills are not available after hours, on weekends or holidays. In case of an emergency, you may need to contact your primary care physician or proceed to the emergency room.
